Cuyahoga County CHNA

To help inform and effectively address the health needs of the communities they serve, hospitals routinely conduct community health needs assessment (CHNA) reports to collect necessary data that will give them a clearer picture of the health concerns in their communities. Though Northeast Ohio hospitals have conducted these assessments for many years, the Affordable Care Act (ACA) included provisions that mandated CHNAs and a corresponding implementation plan at least once every three years for all 501(c)(3) hospitals. Recent regulatory guidance in Ohio has encouraged hospitals to partner with public health departments and other key stakeholders to conduct collaborative CHNAs at the local level that align with state population health priorities described in the State Health Assessment. These reports serve as frameworks for developing implementation plans that describe how all partners will contribute to addressing the most pressing community health needs.

The Center for Health Affairs is proud to have co-led efforts – together with HIP-Cuyahoga co-chair, Cuyahoga County Board of Health Medical Director and Case Western Reserve University Associate Professor Dr. Heidi Gullett – that brought together member hospitals, public health, community stakeholders and community residents to develop the collaborative

2019 Cuyahoga County CHNA. This is the second collaborative CHNA produced in Cuyahoga County, the first was developed in 2018. This work represents an unprecedented level of collaboration among key stakeholders that is continuing as implementation strategies are developed to address the five health priorities identified in the 2019 Cuyahoga County CHNA:
  • Eliminating structural racism.
  • Enhancing trust and trustworthiness across sectors, people, and communities.
  • Addressing community conditions, such as reducing poverty and its effects.
  • Enhancing mental health and reducing substance abuse.
  • Reducing chronic illness and its effects.
