AHA, City and County Health Officials Visit Cleveland to Study Leading Practices in Collaborative Health Assessment and Improvement PlanningOn March 14, the American Hospital Association (AHA) and the National Association of County and City Health Officials (NACCHO) led a listening session with Cuyahoga County hospital, public health and community organizations that have been working to improve community health through collaborative health assessment and improvement planning. Goals of the visit included:

1) Understanding the characteristics of the hospital, public health and community organization partnership for community health improvement planning.

2) Gaining greater insight into what tools and resources are needed in the field to support partnerships that aim to achieve community health improvement.

This work is part of the Partnership for Public Health (PPH) – a joint effort of the Centers for Disease Control and Prevention, AHA and NACCHO – which aims to increase the capacity of hospital, public health and community organization partnerships by identifying and studying leading practices in collaborative health assessment and improvement planning. The PPH team is convening existing local hospital-public health partnerships across the nation that have demonstrated successful approaches to improving health using a foundation of collaborative models of community health well-being.

In addition to The Center for Health Affairs, stakeholders participating in the local event included:

  • Hospitals (Cleveland Clinic, The MetroHealth System, Southwest General Health Center, St. Vincent Charity Medical Center and University Hospitals)
  • Public health departments (Cleveland Department of Public Health, Cuyahoga County Board of Health and the Ohio Department of Health)
  • Other community partners (Better Health Partnership, Case Western Reserve University, Cypress Research Group and United Way of Greater Cleveland)

The alignment of public health and hospital assessments in Cuyahoga County stems from the long-standing work of HIP-Cuyahoga to link clinical and public health.
