Pathways Community HUB Meeting

The Center for Health Affairs will host the Pathways Community HUB Meeting. A HUB is a network of agencies that deploy community care coordinators who reach out to at-risk individuals through home visits and community-based work. Their goal is to identify any risk factors and then address those risk factors. The Ohio Department of Health has identified the Pathways Community HUB model as one of the evidence-based strategies it believes holds the potential to address all three of the priority topics (chronic disease, maternal and infant health, and mental health and addiction) identified in the State Health Improvement Plan.
