2018 Cuyahoga County Community Health Assessment Enters the Homestretch



The partnership between the Cleveland Department of Public Health, the Cuyahoga County Board of Health and University Hospitals to produce a collaborative 2018 Cuyahoga County Community Health Assessment is entering the homestretch. Co-led by HIP-Cuyahoga, Case Western Reserve University School of Medicine and The Center for Health Affairs, the process to develop the 2018 Cuyahoga County Community Health Assessment began in fall 2017 and has been on an aggressive timeline.

The decision to work collaboratively centers around the desire to more effectively address health inequities in Cuyahoga County and to align local community health improvement planning efforts with state population health efforts. Once finalized, the 2018 Cuyahoga County Community Health Assessment will be the first combined assessment of its kind in the county and represents a new, more effective and collaborative approach to identifying and addressing the health needs of the community.

On July 27, 2018, a team of leaders from the hospital and public health sectors in Cuyahoga County met to select the priority health needs that will guide the focus of the community health improvement plan. Leaders involved in this meeting work closely with the community, have an in-depth understanding of community health issues and have grounding in health equity concepts. After reviewing community voice, community stakeholder, hospital, and secondary data for Cuyahoga County – and using established criteria to guide their decisions – five priority health needs were selected for the 2018 Cuyahoga County Community Health Assessment.
